P2RX3,即嘌呤能受体P2X配体门控离子通道3型蛋白,常被称为P2X3受体,别名P2X3。P2RX3属于ATP门控的非选择性阳离子通道家族,主要分布于感觉神经元末梢,通过与胞外ATP结合触发钠、钙离子内流,参与痛觉信号传导及内脏器官的神经反射调控。研究发现,其过度激活与慢性咳嗽、膀胱过度活动症及糖尿病神经病变密切相关,在神经源性炎症和内脏超敏反应中起关键作用。近年来针对P2X3受体的药物开发取得突破,如首个高选择性拮抗剂Gefapixant(AF-219)已完成Ⅲ期临床试验,证实能显著改善难治性慢性咳嗽患者症状,但存在味觉障碍副作用。另有Eliapixant(BAY-1817080)等二代药物通过结构优化降低不良反应,目前处于Ⅱ期研究阶段。该靶点同时被探索用于治疗间质性膀胱炎和神经病理性疼痛,其双重拮抗剂(同时阻断P2X2/3异源三聚体)在肠易激综合征等疾病中展现出潜在治疗价值。
