CHRNA1蛋白全称烟碱型乙酰胆碱受体α1亚基,俗称肌肉型nAChR α亚基,是神经肌肉接头处传递电信号的核心元件。这种跨膜蛋白形成五聚体离子通道,主导运动神经元释放的乙酰胆碱识别,触发骨骼肌收缩反应。CHRNA1功能异常直接导致先天性肌无力综合征,临床表现为眼睑下垂、呼吸肌疲劳等运动障碍,部分突变类型还关联夜间发作性运动障碍。在自身免疫性疾病中,针对该受体的抗体可引发重症肌无力危象。
近年来针对CHRNA1的药物开发聚焦于变构调节技术。新型小分子化合物如GDC-0134通过稳定通道开放状态,在动物模型中成功逆转获得性肌无力症状。基因疗法领域则尝试利用腺相关病毒载体递送正常亚基基因,修复遗传性突变引起的通道功能障碍,目前已完成灵长类动物安全性验证。值得注意的是,某些蛇毒多肽对该受体具有高选择性抑制作用,为开发精准麻醉剂提供了分子模板,但其临床转化仍受限于免疫原性问题。
